We have an open list and warmly welcome requests for registration from patients currently living or moving into the area.
All new patients who wish to register with the practice will need to complete our Registration forms, these are available by from the surgery or see our Form Downloads Page. We DO NOT accept completed registration forms via post. Completed forms must be dropped into The Surgery to a Receptionist.
To help us with your registration, please provide us with your NHS number, proof of address or photo identification.

From the 1st of April 2015, the practice is required under the terms of the latest GP contract to allocate all patients a named accountable GP. This GP has responsibility for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each of their patients where required, based on the GP’s clinical judgement
If you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery. Please inform us if you wish to change your accountable GP.
Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.
